Gastric bypass surgery helps diabetes but doesn't cure it, study suggests

ScienceDaily (Nov. 26, 2012) ? After gastric bypass surgery, diabetes goes away for some people -- often even before they lose much weight. So does that mean gastric surgery "cures" diabetes? Not necessarily, according to the largest community-based study of long-term diabetes outcomes after bariatric surgery. For most people in the study, e-published in advance of print in Obesity Surgery, diabetes either never remitted after gastric surgery or relapsed within five years.

Among the two thirds of the study's patients whose diabetes at first went away, more than a third re-developed diabetes again within five years after gastric surgery. After adding in the one quarter of patients whose diabetes never remitted after surgery, most (56 percent) of the study's patients had no long-lasting remission of their diabetes following gastric surgery. However, when diabetes did go away, the research team extrapolated, it stayed away for a median of eight years.

Which kinds of obese people with type 2 diabetes are likely to get the most benefit from gastric surgery? "Our results suggest that, after gastric surgery, diabetes stays away for longer in those people whose diabetes was less severe and at an earlier stage at the time of surgery," said principal investigator David E. Arterburn, MD, MPH, a general internist and associate investigator at Group Health Research Institute. "Gastric surgery isn't for everyone," he said. "But this evidence suggests that, once you have diabetes and are severely obese, you should strongly consider it, even though it doesn't seem to be a cure for most patients."

The multi-site study tracked 4,434 adults at Kaiser Permanente Northern California, Kaiser Permanente Southern California, and HealthPartners for 14 years: from 1995 to 2008. The research arms of all three of these integrated health care delivery systems -- and Group Health Research Institute, where the study's results were analyzed -- belong to the HMO Research Network. The patients had type 2 diabetes that was either controlled with medication or else uncontrolled, and they were also obese enough to be candidates for gastric bypass surgery.

"Diabetes is an increasingly common disease that tends to keep getting worse relentlessly," Dr. Arterburn said. More than 25 million American adults have diabetes -- and as populations age and keep gaining weight, 50 million are predicted to have it by 2050. Already, diabetes accounts for 5 percent of all U.S. health care spending. And it raises the risk of blindness, kidney disease, heart attacks, strokes, and deaths.

"Prevention is by far the best medicine for diabetes," Dr. Arterburn said. "Once you have diabetes, it's really hard to get rid of. Attempts to treat it with intensive lifestyle changes and medical management have been disappointing." For instance, the National Institutes of Health recently halted the Look AHEAD study of intensive lifestyle changes for people with diabetes. Despite improvements in risk factors like body weight, fitness, and blood pressure, sugar, and lipids, that study showed lifestyle changes did not lower the outcomes that matter most: heart attacks, strokes, and deaths.

"No wonder so many were excited to learn that diabetes can remit after gastric surgery -- even, in some cases, before any significant weight loss -- and many were hoping that gastric surgery might be a 'cure' for diabetes," Dr. Arterburn said. "Our study is the first major evidence that diabetes often recurs after gastric bypass surgery." Still, he added, even after diabetes comes back, having had a long period of post-surgery remission is likely to have many positive effects, such as fewer complications of diabetes: less damage to eyes and kidneys, and fewer heart attacks, strokes, and deaths. The researchers are now funded by the National Institutes of Health to study that possibility in this same population. Dr. Arterburn is also leading a randomized controlled pilot trial of intensive behavioral treatment vs. gastric surgery at Group Health with colleagues from the University of Washington.

It's still not clear whether diabetes relapse happens because of gaining weight back or because of underlying the progression of diabetes. But patients' weight -- before and after surgery -- was not strongly correlated with remission or relapse of diabetes in this population.

As part of the Developing Evidence to Inform Decisions about Effectiveness (DEcIDE) program, the Agency for Healthcare Research and Quality (AHRQ) of the U.S. Department of Health and Human Services funded this project under contract HHSA290-2005-0033-I-TO10-WA1, led by Dr. Arterburn.

Fast forward to the past: NASA technologists test 'game-changing' data-processing technology

ScienceDaily (Nov. 27, 2012) ? It's a digital world. Or is it? NASA technologist Jonathan Pellish isn't convinced. In fact, he believes a computing technology of yesteryear could potentially revolutionize everything from autonomous rendezvous and docking to remotely correcting wavefront errors on large, deployable space telescope mirrors like those to fly on the James Webb Space Telescope.

"It's fast forward to the past," Pellish said, referring to an emerging processing technology developed by a Cambridge, Mass.-based company, Analog Devices Lyric Labs.

So convinced is he of its potential, Pellish is meeting with scientists and engineers to explain the technology's capabilities and is using fiscal year 2013 NASA Center Innovation Fund resources to build printed circuit boards that researchers can use to test the technology's performance for a range of scientific applications. Pellish works at NASA's Goddard Space Flight Center in Greenbelt, Md. He also has carried out preliminary radiation-effects studies to see how the technology's architecture holds up under the extreme environment encountered in space.

"I wouldn't do it unless I really believed in it," Pellish added. "This is one of the few things I've seen that is really different than what others are trying to do. I think this technology could fundamentally change the way we carry out onboard processing."

Analog-Based Microchip

The new technology is an analog-based microchip developed with significant support from the Defense Advanced Research Projects Agency (DARPA). Instead of relying on tiny switches or transistors that turn on and off, producing streams of ones and zeroes that computing systems then translate into something meaningful to users, the company's new microchip is more like a dimmer switch. It can accept inputs and calculate outputs that are between zero and one, directly representing probabilities, or levels of certainty.

"The technology is fundamentally different from standard digital-signal processing, recognizing values between zero and one to accomplish what would otherwise be cost prohibitive or impossible with traditional digital circuits," Pellish said.

The processor's enhanced performance is due to the way the technology works, he explained. While digital systems use processors that step through calculations one at a time, in a serial fashion, the new processor uses electronic signals to represent probabilities rather than binary ones and zeros. It then effectively runs the calculations in parallel. Where it might take 500 transistors for a digital computer to calculate a probability, the new technology would take just a few. In other words, the microchip can perform a calculation more efficiently, with fewer circuits and less power than a digital processor -- attributes important for space- and power-constrained spacecraft instruments, Pellish said.

Although "there has been an overwhelming amount of positive support for the technology within Goddard" since Pellish began introducing colleagues to its capabilities, he is the first to concede that the technology isn't appropriate for all space applications.

Fast Fourier Transform

Because of its efficiency and inherent design, however, it's especially ideal for computing fast Fourier transforms (FFTs), and more particularly the discrete Fourier transform, a ubiquitously used mathematical algorithm in digital-signal processing. Among other things, Fourier transforms decompose signals into their constituent frequencies and are used to generate and filter cell-phone and Wi-Fi transmissions as well as compress audio, image and video files so that they take up less bandwidth.

Among other products, the company has developed an analog-based integrated circuit geared specifically for computing Fourier transforms. The team will use the technology, which the company donated, to assemble several custom circuit boards. "We'll take the hardware and see what it can do with our data and applications," Pellish explained.

One of the first applications the group plans to target with a version of the FFT integrated circuit is wavefront sensing and control, the computational technique for aligning multiple mirror segments, like those that are flying on the Webb telescope, so that they operate as a single mirror system.

In addition, Jeffrey Klenzing, who works with Goddard's Space Weather Laboratory, wants to evaluate the technology's use for on-board data processing, particularly for studies of the sun. "For a typical sounding rocket application, we send all data down and perform fast Fourier transforms on the ground. However, for satellite missions, this is not feasible given limited telemetry," Klenzing said. "A chip for performing rapid, reliable FFTs would be very useful for such heliophysics missions particularly with the push toward smaller, low-power satellites such as CubeSats and nanosats."

Pellish also believes autonomous rendezvous and docking and other applications requiring precise locational information would benefit from the analog-based technology. "We're trying to create a new market at NASA for analog processing. I believe it will give us a competitive edge. If we can push this, it could revolutionize how we do onboard data processing."

HBT: Mets' Dickey surprised by slow pace of talks

While making an appearance at a school in Harlem to promote literary R.A. Dickey gave an update on his contract talks with the Mets, saying that ?the pace is slow? and reiterating that he won?t continue to negotiate once the season begins.

Dickey is under contract for $5 million in 2013 and has said all along that he won?t talk in-season, making it clear that he?d like to get an extension worked out sooner than later. He also noted that he thought talks would proceed more quickly based on what general manager Sandy Alderson said early in the offseason.

?We have until spring training, but I would think they would formulate some kind of plan so they can know what they can do forthcoming,? Dickey said, via Adam Rubin of ESPN New York. ?They have offered and we have countered. That?s kind of the stage that we?re in. We?re hoping to hear back from them soon and get the dialogue going again.?

Zillow Acquires Rental And Real Estate Search Site HotPads For ...

Zillow just announced that it has acquired the San Francisco-based rental and real estate search site HotPads for $16 million in cash. The deal is expected to close in the fourth quarter of 2012. This acquisition marks the first time that Zillow has acquired a primarily consumer-facing company and Zillow says it is making this acquisition to ?expand the size of its growing rental audience and extend the reach of its marketing tools and productivity solutions for rental professionals.?

HotPads has been around for a while. The company launched in 2005, with a focus on rentals. It expanded to include real estate sales in 2007 and started including vacation rentals and hotels in 2009. The company, which competes with the likes of Trulia, Apartments.com, Craigslist and, of course, Zillow itself, raised $2.3 million in a Series A round led by Meakem Becker Venture Capital in 2007, but hasn?t raised any funding since. The company has 19 employees and will continue to operate from San Francisco and join the Zillow Rental Network once the acquisition has closed.

The basic idea here, as Zillow CEO Spencer Rascoff told me earlier today, is to create one single database for rentals that will be available on Zillow, HotPads and the company?s other properties. HotPads, he stressed, will continue to exist as its own brand and Zillow will commit additional engineering resources to help the company grow.

HotPads, Zillow tells us, attracted about 2.8 million unique users in October. Nearly 70% of the listings these users looked at were apartments for rent. On Zillow, about 6 million users look at rentals each month according to the company?s own data.

Between Zillow Rentals and HotPads, Rascoff expects, the company can now offers a rentals marketplace at scale ? something that wasn?t previously available to users in the highly fragmented rentals market. For Zillow, this is also an opportunity to build a relationship with a younger audience ? the kind that?s shopping for a first rental apartment but not ready to buy yet. Once they get to the point where they are looking to buy, they will hopefully come back to the Zillow brand.

?This acquisition represents a significant step-change for Zillow Rentals, allowing us to dramatically increase the number of leads we send to landlords. HotPads has a younger, complementary and rental-focused audience. Now Zillow will become even more relevant to consumers at the beginning of their real estate life cycle,? said Spencer Rascoff, CEO of Zillow in a canned statement today. ?In addition, by acquiring an amazing engineering team, with a deep understanding of how people search for rentals and become tenants, we expect to accelerate our innovation and monetization of our rental marketplace.?

Zillow, of course, has lately made a number of investments in the growing online retail market, including the recent launch of its free marketplace with tools for rental professionals that was largely based upon the company?s June 2012 acquisition of rental relationship management service RentJuice. For Zillow, this is the sixth acquisition in the last two years. Just this month, the company announced its plans to buy mortgage technology company Mortech. In October, the company acquired Buyfolio and in June it acquired RentJuice. In 2011, it acquired Postlets and Diverse Solutions.

End-of-year engineering school projects often pique our interest for their creativity. It's not every day that they can carry a bassline, however. James McVay's robot project for his honors year at the Victoria University of Wellington, the supremely well-named MechBass, wouldn't have much trouble keeping up with a favorite band. It centers on a custom, Arduino-compatible board that controls the plucking, fretting and damping of four strings to faithfully recreate bass guitar sounds from MIDI input. The design even accounts for the unwanted noises of actuators and motors, while virtually everything was either 3D-printed or laser-cut just for the task at hand. Sounds good? There's more in the pipeline: an upcoming Swivel robot will experiment with different playing techniques, and McVay ultimately sees his work teaching us about robotic music's interaction with human performers. For now, we'll be happy with the video after the break and hope that MechBass takes requests.

Gabriel Aubry, Halle Berry Ex, Arrested After Thanksgiving Fight With Her Current Boyfriend

LOS ANGELES ? Halle Berry's ex-boyfriend Gabriel Aubry was arrested for investigation of battery Thursday after he and the Oscar-winning actress's current boyfriend got into a fight at her Hollywood Hills home, police said.

Aubry, 37, was booked for investigation of a battery, a misdemeanor, and released on $20,000 bail, according to online jail records. He's scheduled to appear in court Dec. 13.

Aubry came to Berry's house Thanksgiving morning and police responded to a report of an assault, said Los Angeles Police Officer Julie Boyer. Aubry was injured in the altercation and was taken to a hospital where he was treated and released.

Emails sent to Berry's publicist, Meredith O'Sullivan, and Aubry's family law attorney, Gary Fishbein, were not immediately returned.

Berry and Aubry have been involved in a custody dispute involving their 4-year-old daughter, Nahla. The proceedings were sealed because the former couple are not married. Both appeared in the case as recently as Nov. 9, but neither side commented on the outcome of the hearing.

Berry has been dating French actor Olivier Martinez, and he said earlier this year that they are engaged.
